Complete gallbladder torsion diagnosed with sequential computed tomography scans: a case report

چکیده
INTRODUCTION Torsion of the gallbladder is an extremely rare cause of acute abdomen, which commonly affects thin elderly women. A prompt surgical approach is necessary to avoid fatal complications associated with gangrene and perforation of the gallbladder. However, it is difficult to make a preoperative diagnosis using ordinary imaging modalities. CASE PRESENTATION An 84-year-old Japanese woman was admitted to our hospital due to left lower abdominal pain. Her pain shifted suddenly to the right upper abdomen a half day after admission. Although her enlarged and wall-thickened gallbladder had been already seen at admission, it rotated approximately 180 degrees and deviated to the midline of her abdomen on the second computed tomography scan, which helped us to make a correct diagnosis of gallbladder torsion. The patient underwent an emergency operation (detorsion and cholecystectomy) and recovered without any complications. The gallbladder had necrosis due to torsion. CONCLUSION Sequential diagnostic imaging might be helpful to make a preoperative diagnosis of gallbladder torsion when the gallbladder is enlarged and wall thickened but the patient does not present with typical clinical symptoms.

Gallbladder Torsion with Accompanying Acute Appendicitis
Gallbladder torsion is a rare clinical entity and it is a difficult condition to diagnose preoperatively. About 500 cases of gallbladder torsion have been reported since 1898, when Wendel first described gallbladder volvulus. This condition most commonly occurs in elderly women and the symptoms of this disease are largely non-specific and they mimic those of acute cholecystitis. Even with the r...

INTRODUCTION Torsion of the gallbladder in the pediatric population is rare. A nonspecific clinical presentation is characteristic, which frequently contributes to diagnostic uncertainty. CASE DESCRIPTION We report a case of gallbladder torsion in a 6-y-old male with abdominal distention and lethargy accompanied by a low-grade leukocytosis. An abdominal computed tomography scan suggested the ...

Torsion of the gallbladder is a relatively rare surgical disease which mimics acute cholecystitis' symptoms that usually would not be respondable to medical therapies. The torsion would mostly be diagnosed intra-operatively. Cholecystectomy is the treatment of choice. متن کاملThe Prevalence of Unanticipated Hamate Hook Abnormalities in Computed Tomography Scans: A Retrospective Study
Background:It is possible that some hamate hook fractures are not diagnosed or treated, thereby affecting the study of their natural history. Study of the prevalence of incidental hamate hook fractures, nonunions, and other abnormalities on computed tomography (CT) ordered for another reason could document a subset of undiagnosed and untreated hamate hook fractures which might change our under...
متن کاملGallbladder Torsion: A Rare Cause of Acute Abdomen
Gallbladder torsion is one of the important causes of acute abdomen. Early diagnosis and treatment is important because surgical intervention is required. Abdominal ultrasonography (US) and contrast-enhanced computed tomography (CT) are useful diagnostic tools prior to surgery. Here, we present the case of an elderly woman who underwent emergency cholecystectomy due to gallbladder torsion. Rece...
